Procedure
- Check title reflects the original specific need and is clear to understand
- All contact details, telephone numbers, contact person, email addresses are CORRECT
- Consent forms have been signed if patient or staff photographs are used
- Images/diagrams are copyright approved and captioned and attributed to their source
- All logos are correctly placed and SWSLHD brand applies to your hospital
- Ensure only 2 colours are used, usually black and one primary colour and are suitable for both white or coloured paper readability
- Use the Feedback Form for written patient information to capture all necessary feedback evaluation for consumers
- Pilot your resource with 5 or more patients/consumers.2 staff/peers with no knowledge of the topic discussed.
- Take to Consumer board???
- Review feedback and adjust resource as required.
- Ensure an author (personal or team) is on the new resource with a publication and review date included, (review date no more than 3 years)
